Papillary Cystic Neoplasm


Abstract
In contrast to the more common form of pancreatic neoplasm, the papilliary cystic neoplasm has been described recently that would appear to have a favorable prognosis. It occurs most often in adolescent girls and young women otherwise healthy.
Most
of
the tumors were located in the body or tail of the pancrease. The incidence is probably about 1% of pancreatic tumors The tumor is capable of local invasion but usually does not metastasize. Recurrence following surgical resection was rare.
Recently authors experienced three cases of this neoplasm in the Department of Surgery, Chonnam University Hospital. In all cases, the tumors occured in healthy young women 12, 16 and 23 years respectively. All of them presented with abdominal
mass. Two
occurred in the body of the pancreas and another in the head.
Gross findings revealed apparent encapsulation, solid portion, cystic component with hemorrhage. Microscoplcally. solid and papillary patterns with variable degree of cystic degenerations were found. Immunohistochemical staining was positive for
neuron-specific enolase (two of three cases) and a1-antitryupsin (all cases) in the solid and papillary portion of the tumor. Electron microscopic examination was performed on one case and revealed acinar and islet cell differentiation of the
tumor.
Distal pancreatectomy and splenectomy was performed in two cases and local excision in one case. All of the patient presented here are alive and well for two to three years postoperatively.
